Work or school absence notes are available to current patients (patients who have been in the clinic within the past 12 months) at no fee.

For others requesting absence notes, a short health evaluation will be performed and the work or school absence note (or if needed, a phone call from Dr. Pat) will be provided for a fee of $25.
